I was thinking mange, but would try the antibacterial shampoo first.

When Loki's seasonal allergies get really bad combined with him blowing his coat for the Winter he gets random little bald patches too. He has been to the vet for it in the past tested for mange and everything but each test comes back clean with nothing wrong. If you look closely can you see new fur coming in the bald area?

Hmmm.. I am not sure then we can also see little new fur coming in a couple days after we notice the spots.

My Bella would get like that... we had her tested to see if it was mange, but it was just seasonal alopecia.
