This actually doesn't look all that bad. My Lincoln has a HUGE nose rope, and it does require attention. I usually wipe it clean 2-3x a week with Witch Hazel and then top with diaper rash ointment.
Well, I spoke too soon :( I was hopeful in seeing improvement, but after a full month of antibiotics, medicated baths 2-3x a week (with bleach rinse), he has no improvement. See pictures below.
Back to the drawing board I suppose. I am going to give him some time off from meds for now and then...

Just a quick update - it's been almost 1 week on Cephalexin and medicated baths 3x a week. I believe he is showing signs of improvement. The spots are shrinking in size and it appears the hair is slowing come back.

I just received a call back from the vet today. They ran a culture and it is a bacterial infection (not yeast related). They wanted to do a culture test to determine which antibiotic would be the most effective. He will be on cephalexin for the next 30 days, along with chlorhexidine gluconate...
I think staph is definitely the consensus, including my breeder. She recommends cephalexin for 4-6 weeks, coupled with chlorhexidine baths. I am taking him in on Saturday for evaluation/treatment. In the meantime, I will start adding some ACV/yogurt/salmon oil to his food and continue giving him...
